The continuous improvement in MRC technology (e.g. the use of higher magnetic fields) means that the likelihood of an abnormal ERC in patients with normal MRC is small. If MRC is normal and PSC is suspected, it is reasonable to consider patient referral to centres with known technical expertise in MRC as a first step. These results suggest that small duct PSC is a distinct clinical entity and in the majority of cases, it does not represent an early form of classical PSC. Further prospective studies are required in order to determine the status and role of ANCA and in particular whether immunogenetic studies will reveal any further differences between small and large duct disease. Small-duct PSC may recur after liver transplantation [5]. Ursodeoxycholic acid (UDCA) is widely used in the treatment of a variety of cholestatic liver diseases, but very limited data exist on its effects in small-duct PSC [24]. No controlled trials of medical therapy including only patients with small-duct PSC have been performed.

In a study reported in the November issue of Gut, investigators compared outcomes over eight Background/Aims: This study aims at describing the natural history and outcome of small duct primary sclerosing cholangitis (PSC). Methods: Thirty-two patients with small duct PSC were studied. The average time taken for diagnosis was 69 (1-168) months.
This means it is a disease that affects the bile ducts – tubes that deliver bile from the liver to the small intestine. PSC occurs because of inflammation in the bile ducts (cholangitis), which results in hardening (sclerosis) and narrowing of the ducts 1 .
2017-12-01 BACKGROUND: Primary sclerosing cholangitis (PSC) is a chronic progressive cholestatic liver disease, which usually affects young adults and is diagnosed by cholangiography. On a few occasions, the disease either starts in or exclusively involves the small intrahepatic bile ducts, referred to as small-duct PSC. Small duct primary sclerosing cholangitis is a variant of PSC in which affected individuals have abnormal blood tests and a liver biopsy that reveals features that are consistent with a diagnosis of PSC. However, these individuals have normal findings on more sensitive tests such as magnetic resonance cholangiopancreatography. Small-duct PSC, a variant of PSC, also has an association with inflammatory bowel disease.
6-10 % der PSC-Fälle aus. Das mediane Alter bei Diagnosestellung beträgt etwa 38 Jahre. Small duct PSC show a more protracted course than large duct PSC [7, 12], but the number of patients with small duct AISC is too small for a prognostic evaluation. However, one may speculate that the AIP type 2-like liver damage signifies a better response to steroids compared to PSC in general. Small‐duct PSC represented 5.8% of patients (18 of 309) with sclerosing cholangitis. Subsequent endoscopic retrograde cholangiography (ERC) performed in 5 patients with small‐duct PSC showed progression to typical PSC in 3 patients at 4, 5.5, and 21 years of follow‐up.

The macroscopic bile ducts in small duct PSC are normal; hence, cholangiography will not be revealing. Diagnosis of small duct PSC requires liver biopsy, to demonstrate the characteristic microscopic disease.

Sometimes someone has normal imaging but presents with the symptoms of PSC and/or liver function tests consistent with PSC (that can't be explained by another liver disorder) and/or a liver biopsy consistent with PSC. These people are diagnosed with small-duct PSC. Conclusions: Small-duct PSC is a disease of progressive potential but associated with a better long-term prognosis as compared with large-duct PSC. Primary Sclerosing Cholangitis Symptom Itching. Depending upon the underlying cause, itchiness may be associated with other symptoms and signs. Most commonly, these associated findings include skin lesions such as rash, blisters, bumps, or redness of the affected area.
